13 | On 03/03/2025 Licensing Program Analyst (LPA) Cristina Castellanos arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegation. LPA Castellanos met with Director Dawn Speciale and discussed the purpose of the visit. LPA Castellanos toured the facility both indoor and outdoor and observed (4) children in care with one (1) adult staff providing care and supervision.
During the investigation, LPA Castellanos observed and reviewed all documents posted in a prominent, publicly accessible area at the infant facility. Furthermore, LPA observed the Notification of Parents’ Rights (PUB 393) posted on the Parent Board to have the incorrect Licensing Office address and telephone number.
Per LPA record review it was found that the facility has the incorrect postings for parents/guardians. Based on LPA record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.There was one deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1.
