32 | During the interview with the Director, it was disclosed that on 1/25/24, kitchen staff disclosed to management that there were rodent droppings found in the kitchen, near the refrigerator. An emergency work order was placed and a visit was conducted by an exterminator from Western Exterminator Company (WEC), on 1/26/24. According to the invoice from WEC, due to possible droppings found in the kitchen closet, traps were set with versa boxes in the kitchen area. 1 was set in the closet, 1 behind the fridge and 1 besides the oven. Another versa box was also set in the pantry room. Per the Director, she observed rodent droppings on 1/29/24 and the entire kitchen area (including behind the fridge and stove) were cleaned and sanitized. All of the items on the kitchen shelves, such as foods and paper goods were discarded.
Statements obtained from staff were consistent and corroborated with the statement obtained from the Director. Per staff, rodent droppings were found in the kitchen by a staff on 1/25/24, at around 10AM (pictures obtained). On 1/26/24, an exterminator from WEC came to the center and set traps in the kitchen area and pantry room.
During today's visit, LPA Katrdzhyan toured the kitchen area and observed 2 rodent traps in the kitchen and 1 in the pantry room. Based on the information gathered, there is sufficient evidence found to prove this allegation to be true.
Based on LPA’s observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, (Title 22, Division & Chapter number), is being cited on the attached LIC 9099D.
The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
