25 | On 2/15/24, at 1PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case
Management Visit to this facility. Upon arrival, LPA met with Cheryl Gartsman, who assisted with the visit. There were 48 children with 6 staff observed present.
LPA Katrdzhyan discovered that the facility failed to report an incident to Community Care Licensing (CCL), which took place several months ago (date unknown), involving Child 1 and Staff 1. During an interview with the Director / Cheryl Gartsman, it was confirmed that an Unusual Incident/Injury Report (LIC 624) was not submitted to CCL, per Title 22 Reporting Requirements, Section 101212.
During today's visit, the Director was asked to submit an Unusual Incident/Injury Report regarding the incident involving Child 1 and Staff 1 to CCL, by no later than the end of business day on 2/15/24.
The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22.
The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
