32 | On 06/15/23, according to the Face-to-Name Transition sheet, the Infant classroom 1 was out of ratio from 8:43 AM to 9:54 AM. The Director had to assist in the infant classrooms from 8:59 AM until 10:52 AM because the infant and preschool did not have sufficient teachers to meet minimum ratio requirements and there was no other staff that could provide assistance in the classrooms. During this time the facility continued to accept children and there were no staff at the front desk to direct parents to their children’s classrooms. The combination center did not have any staff available that could relive a teacher from their respective classrooms if for any reason they had to step out. The Director informed LPA Almanza that Staff 5 had called in and would come to work as soon as S5 was available but could not provide an exact time.
On 06/15/23 at 9:13 AM, LPA observed the Director with 3 infants inside the Buggy, in front of the Directors office. The director was greeting parents and directing children to their classrooms. At 9:29 AM LPA observed 4 infants in the buggy. At 9:38 AM, LPA observed the Director in room 5 with Staff 12 and 8 infants, Director stated they had been in the room for 5 or 6 minutes.
On 06/14/23 t at 7:36 AM, LPA entered the combination center to conduct an unannounced sit visit. Upon arrival, LPA observed Staff 5 and 3 infants sitting in the infant “Buggy” located in the lobby between the Directors office and the front desk. According to the Face to Name sign in sheet, the 3 infants arrived between 6:37 AM and 7:24 AM and were taken into their classroom after LPA arrived. LPA observed staff 5 greeting parents in the lobby and directing them to their children’s classrooms.
Staff 5, 11 and 12 are reporting that during drop off, infant children are placed in the buggy located in the lobby. At 8:00 AM, infant children are picked up from the buggy and taken to their classrooms, LPA Almanza discovered that this is a regular occurrence.
During todays visit 06/29/23 at 8:04 AM, upon arrival to the facility LPA observed 4 infant children in the Buggy with the Director and Assistant Director. The Director stated that they are allowed to keep infant children for 20 minutes at a time in the buggy. Assistant Director stated that infants had just arrive and had not been long in the buggy. LPA asked for the Face-To-Name transition sheet and Assistant Director stated it was in the classroom and that she had just relived Staff 1 who was supervising infants. Upon reviewing the Face-To-Name, LPA observed that Child 1 arrived at 6:45 AM, Child 2 arrived at 7:40 AM, and Child 3 and 4 arrived at 7:56 AM. LPA Interviewed staff 1 who reported that she was supervising the infant children and that as they arrived they were placed on the buggy. Page 2 of 3
