Licensing Program Analyst (LPA) Alicia Bailey conducted an unannounced complaint inspection on 02/25/22 for the purpose of delivering the findings for the above allegation. LPA arrived at the facility at 1:00 PM and met with director Sherry Lenard. Director Sherry Lenard gave LPA a tour of the facility. There were 23 children and 6 staff present.

During this investigation, LPA Bailey collect children roster, and other pertinent documentation. LPA Bailey reviewed staff records and observation. Interviews were conducted with owner, director, staff, parents and children. LPA interviewed owner, director and staff who deny the allegations. Staff interviewed did not make any corroborating statements. LPA Bailey interviewed parents and children. There were no corroborating statements made. Although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore at this time the above allegations are unsubstantiated.

Exit interview was conducted with director Sherry Lenard . The Sherry Lenard was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
