On February 21, 2024 at 12:00 pm Licensing Program Analysts (LPAs) A. Wallin and T Tran arrived at the above licensed facility. While conducting a complaint investigation, LPAs observed the following deficiencies: 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

During tour of outdoor facility, LPAs observed two outside gates open easily accessible for children in care. LPAs observed one purple gate leading to back of facility with broken latch. LPAs observed one red gate serving as entrance to facility without any self-latching security. LPAs obtained photos for record. A type B citation issued.

Per licensee, she agreed to have the gate fixed with proper latching and to submit picture evidence in order to clear citation by POC date: 3/6/2024. Exit interview conducted and report was reviewed with the licensee, Tracie Adams. A notice of site visit was given and must remain posted for 30 days.

This requirement met based on observation of facility failed to ensure safety of well kept facility gates. LPA observed the entrance to the facility gate and side gate did not have propoer latch which poses potential health and safety risk to children in care.
