On 4/17/2024 at 10:50am, Licensing Program Analyst (LPA), Loyce Phillips arrived to the facility to conduct a Plan of Correction visit and was met by both Licensee Fidencia and Chris Jimenez. LPA observed 8 children and 1 infant in care with both Licensees and 2 Assistants. All adults have a criminal record clearance.

On 2/22/2024, Licensee was cited for the following:
1. Staff did not have current Mandated Reporter Training Certificates on file.
2. Children files were missing LIC 995A.

During visit LPA observed the following:
1. Staff have completed Mandated Reporter Training.
2. LPA reviewed a sample of children files and observed LIC 995A.

Citations issued on 2/22/2024 has been cleared.

An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Licensees.
