25 | On 3/25/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted a case management visit at facility mentioned above for the purpose of investigating a self reported reporting requirements incident. According to the Unusual Incident Report, the rear playground at the facility will undergo renovations. LPA met with Site Supervisor, Marlene Yacou and toured the facility indoor and outdoor. During this visit, LPA observed 21 children being supervised by 5 staff.
LPA made observations, obtained documentation and conducted interviews and obtained pictures of the play area under renovation.
The Department received the Unusual Incident Report via email on 3/13/2025. According to the email, Playground will undergo renovations beginning 3/18/2025.
During todays inspection, LPA observed children playing in the front yard area of the center. The front yard area is completely gated, and has a door that locks. LPA observed children were supervised accordingly in the outdoor area. LPA also observed tables/chairs and toys to play with. LPA observed water readily available for children. LPA observed the backyard area to be inaccessible to the children in care. The doors leading out to the backyard play area have doorbells that ring when opened. There is caution tape placed around around the patio rails which prevents anyone to walk beyond the small patio area and into the main play area under renovation. Children remain under supervision of a teacher at all times.
Per Site Supervisor, Parents were verbally notified upon drop off and pick up on 3/12/2025 and an update sent out to all parents via Learning Genie on 3/25/2025. The renovation is scheduled to be concluded on or before April 11, 2025. An inspector will inspect the yard to ensure it is safe for children before children are allowed to utilize the play yard.
No deficiencies are being cited during todays visit. An exit interview was conducted and a copy of this report and Notice of Site Visit were provided to Site Supervisor, Marlene Yacoub. |
