Licensing Program Analyst (LPA) Sabrina Martinez conducted an unannounced annual inspection to the above facility on 12/06/2022. LPA arrived at the facility identified self and met with Lead Teacher Liz Calderon who guided analyst on a tour of the facility. LPA provided Lead Teacher with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection.

This is a preschool program which consists of 1 classroom; RM #1. Facility operation hours are Monday to Friday from 7:50 AM to 10:50 AM (AM Session) 11:40 AM to 2:40 PM (PM Session).

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 1: Staff #1, #2 and #3 with 7 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Per Lead Teacher, the isolation area is located in the nurse’s office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.



LPA discussed the safe sleep regulations with Lead Teacher and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Lead Teacher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Lead Teacher states that poisons are locked in the classroom cabinet. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

LPA observed Lead Teacher’s CPR and Pediatric First Aid certification expired on 01/2022. Staff #2 and Staff #3 unable to show LPA proof of CPR and Pediatric First Aid certification during this inspection.

LPA did not review children and staff files during the inspection due to the files being stored at the office and office manager was out for an entire week. Lead Teacher stated that office manager is the only person who has access to the children and staff files. LPA will review files at a later time and date. If files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 11/2022.

Menus are posted where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides breakfast and lunch. First Aid supplies were observed in the classroom in a cabinet. According to Lead Teacher, medication is only administered to a child when accompanied with a doctor's note and is stored in the nurse’s office. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is in the nurse’s office.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the Lead Teacher to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: LPA provided the facility representative with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.

Based on LPA's observation and interview with Lead Teacher Liz Calderon, the facility did not comply with the section cited above. Lead Teacher’s Pediatric first aid and CPR expired on 01/2022 which poses a potential health, safety or personal rights risk to persons in care.
