On 07/16/2025, Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced Annual Required Inspection for the facility named above. LPA Castellanos was greeted by Director Sheela Deraniyagala. LPA toured the facility both indoors and outdoors and observed a total of 17 children in care, being supervised and cared for by 3 staff members, including the director. Present during today’s inspection was Director Sheela Deraniyagala.

Capacity and limitations as specified on the license are being maintained.

All children are under supervision, including visual supervision, of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. Parents sign their child in/out of the facility. LPA Castellanos advised the Director to ensure that parents sign using their full legal name and the time of day. The preschool hours of operation are from 7:30am to 5:00pm.

Per the Director the preschool center does not provide Incidental Medical Services (IMS).

There are no swimming pools, ponds, or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, and other hazardous items are made inaccessible. All poisons are kept in a locked cabinet area. The facility’s staff members do the day-to-day cleaning in the classrooms.

LPA observed two (2) fully charged fire extinguishers and a carbon monoxide detector throughout the facility. Additionally, the entire facility has a Pull Fire Alarm system. LPA observed the First Aid Kit inside the kitchen area.

All toilets and hand washing facilities are in safe and sanitary operating condition. LPA inspected the preschool classrooms and observed age-appropriate toys, books, and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb all falls.

LPA Castellanos conducted a Health and Safety inspection of the kitchen, and all kitchen food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Solid waste storage containers have tight-fitting covers and are in good repair.

Per the Director the facility only provides snacks and parents assure they provide meals for their children which comply with dietary restrictions and allergies. Drinking water is available to the children in care. Additionally, children bring in their own refillable water container.

The facility communicates with families via flyers and/or verbal communication during drop-off and/or pick-up time.

LPA reviewed six (6) children’s files and observed all files to be in compliance. LPA provided the administrator with the Child Care Center Provider Requirements, LIC125 and the LIC311A for further review and reference.

LPA reviewed four (4) personnel files and observed staff member S1's file to not be in compliance. S1’s personnel file was missing the following: LIC503 Health Screening Report, LIC9108 Statement Acknowledging Requirement to report Child Abuse, Proof of Immunization Record, and TB Clearance. At least one person is trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. LPA discussed the importance of making sure that all Pediatric CPR and Pediatric First Aid training is EMSA approved.

Furthermore, LPA Castellanos reviewed of the Guardian Background Check System confirmed that S1 does not have a current criminal record clearance associated with the facility. Guardian Background Check System search results provided the following message: No applicants that match your search were found in the background checking system. LPA Castellanos additionally requested if a copy of form LIC 9163 Request for Live Scan Service - Community Care for uncleared S1 could be provided. The director and S1 were unable to provide a copy of the Live Scan form. Director Deraniyagala and the unclear staff member (S1) both confirmed he has been employed for more than 5 days.

Based on observation, record review and information obtained an uncleared Staff member (S1) was providing care and supervision to children in care during today’s inspection until 10:37am. The facility is not complying with the Health and Safety Code Section 1596.871(c)(1)(A) …a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, …prior to employment, residence, or initial presence in the facility.

All adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). CCC completed testing and has Remediating Lead Exceedances. LPA Castellanos verified that the lead testing was completed in accordance with Written Directives outlined in PIN 21-21.1-CCP.

LPA reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there were two (2) deficiencies cited during today’s inspection; 1 Type A & 1 Type B. See LIC 9099-D for additional information. As well as a Civil Penalty Assessment - Caregiver Background Check was given.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

An exit interview was conducted, and Plan of Corrections were reviewed and developed with the administrator. A copy of this report and appeal rights were discussed and left with the administrator, whose signature on this form confirms receipt of these documents.
