Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced site visit on 12/18/2024 at 8:25 AM to this facility to deliver findings on the above-mentioned allegations. Upon arrival, LPA met with Administrator, Janice Garris and explained the purpose of the visit. There were seven (6) children and 4 staff were observed at the time of visit.

During the course of the investigation, interviews were conducted and copies of Children's roster were obtained and reviewed.

Per Reporting Party, Day care child sustained unexplained injury(s) while in care. Per interviews with staff if a child obtains an injury while is in care it is documented on a boo boo report and the parents are informed, both the teacher and parent sign the report. The staff also use an app called Remind to inform the parents about injuries or incidents that occur at the school.

Parents interviewed did not present concerns related to the above-mentioned allegations and were pleased with the services and care being provided to their children. Parents stated their children do obtain injuries at the school but they are always informed about the injuries or any incidents.

Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.

The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Administrator Janice Garris and Appeals Rights provided.
