25 | On 3/12/25 Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named facility to conduct an announced Case Management Deficiencies. LPA was greeted by director Larresha Alexander. LPA observed 62 children in care being supervised and cared for by 7 fingerprint cleared staff. LPA toured the facility inside and outside for health and safety.
The purpose for the visit today is criminal records clearance review. S1 began working at the facility on August 26, 2024. Upon review of guardian background electronic data system, S1 had fingerprints that processed for clearance on September 30, 2024. This indicates that S1 was working in the facility prior to fingerprint clearance. Type A citation cited. See LIC 809D. Civil Penalty assessed. See LIC 421BG.
LPA Ranita Richmond informed director Larresha Alexander that this report dated 3/12/25 documents one Type A citation. One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Ranita Richmond informed the director Larresha Alexander to provide a copy of this licensing report dated 3/12/25 that documents one Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted, a copy of this report, and appeal rights were discussed and provided to director Larresha Alexander.
Notice of Site Visit was provided and required to be posted for 30 days.
