Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 Year inspection on this date. LPA met with Nune Begzadyan, Director, and explained the purpose of the inspection and was guided on a tour of the facility. Facility is currently licensed for a capacity of 40 children. LPA observed twenty-four children and three teachers (including Director) present at the time of the visit. Sign in/out sheet confirmed this to be accurate. All children are under supervision, including visual supervision of a teacher at all times. The facility operates Monday through Friday, from 7:00 a.m. to 6:30 p.m. Facility is operating within capacity limitations. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Required posting were observed in classrooms for public viewing. Facility has a working phone land line available. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. The facility consists of three classrooms. A walk through of the classrooms was conducted. The classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Isolation area for sick students is in the Director’s office. LPA observed adequate arrangements for isolation and care of ill children.

The preschool classrooms were observed with one bathroom containing three toilets and three sinks. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate. Furniture and equipment were inspected for age appropriateness and good repair. All rooms are clean and safe. Heating, lighting, and ventilation are adequate. All rooms have central air conditioning and heat. There are cubbies for children's belongings used to store personal items. LPA observed age appropriate toys and materials. Drinking water is available inside the classrooms and outside in the form of pitchers and disposable cups. LPA observed a sufficient quantity of cots available for napping children in the classrooms. Fire extinguisher was observed with a date of service 7/26/2024. Carbon monoxide detector was observed in the kitchen. Trash cans were observed with appropriate lids. First aid kit was observed to have: bandages, adhesive tape, scissors, tweezers, and antiseptic solution. All medications are stored in the directors office and when needed in refrigerator located in the kitchen. Full Kitchen was observed for preparation of meals. Menu's were observed in classrooms. Kitchen was in good repair. The kitchen is equipped with two refrigerators, freezer, and stove. Snacks are stored in air tight containers in the kitchen. The facility provides three snacks and three meals daily. Menus are posted in each classroom. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food and there is hot running water in the kitchen. The preschool kitchen maintains a list of children with allergies. Outdoor play area was inspected. The backyard has 2 sections. A grass area with a swing and slide set and a concrete area for bicycle play. There is also a grass area in the front of the facility which may be used for outdoor play activities. Page 2

The entire property, including all outdoor play areas is fenced and meets regulations. LPA observed a patio area in the rear yard for shade and rest. Licensee has pitchers for available drinking water.
Disinfectants and cleaning solutions were observed to be inaccessible to children in the kitchen. Poisons are locked in the staff bathroom under the sink cabinet.
CPR, Pediatric First Aid and Mandated Reporter Training certificates were observed to be current. The name of the child care center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff and children files. Four out of ten children's records reviewed were missing Physician's Report (LIC701) and staff files missing the immunization records.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
