25 | On December 20, 2022, at 1:00p.m., Licensing Program Analyst (LPA) Denise Gibbs conducted an case management inspection and met with Director Paige Solomon. LPA disclosed the purpose of the inspection and was granted entry into the facility by office staff.
At 1:10 p.m. during an inspection of the preschool program at the facility, LPA observed that the School Age program has children that are non-ambulatory, using a wheelchair. There were 16 children and two staff present during inspection.
The School-Age program is on the second floor of the property. In order to access the second floor you have to go up 15 steps. School-Age restrooms are also on the second floor. LPA notified Director that as of today 12/20/22, no Non-ambulatory children can use the upstairs classroom or restroom. On the first floor LPA did observe a ramp from the parking lot entrance that goes up to a Preschool classroom that is not in use and the same ramp goes down to a preschool classroom that is also currently not in use. LPA observed that the staff restroom on the first floor is ADA accessible with hand rails. As of today, the School Age Program will split their group. Half of the children will remain upstairs and the other half, including the Non-ambulatory children, will utilize one of the Preschool classrooms that are not in use and the staff restroom until further notice. School Age children will not co-mingle with the preschool children. Director completed a Declaration stating that Non-Ambulatory children will not utilize the upstairs classroom or restroom.
No deficiencies will be cited today 12/20/22
A notice of site visit will be given and must remain posted for 30 days.
Exit interview was conducted. A copy of this report and appeal rights were discussed and left with Director, Paige Solomon, whose signature on this form confirm receipt of these documents. |
