25 | Licensing Program Analysts (LPAs) Susann Sanchez and Ashley Calderon conducted an unannounced case management - other. LPAs met with Teacher Denisha White, LPAs where at the facility conducting a complaint investigation.
At 2:00pm, during interviews with staff and children. Staff #1, Staff #2, Staff #3, Staff #4 and Child #1 stated that sometimes children co-mingling. Child #1, stated that when children get in trouble they will go and sit in the infant room. Staff #1 and Staff #2 stated that sometimes children sit in the infant room to calm down. Staff #3 and Staff #4 stated that school- age children play in the preschool classroom. All staff interviewed stated that all age group co-mingle but that it happens rarely. Type A was cited.
At 2:45pm, LPAs gave consult regarding co-mingling and the departments Technical Support Program (TSP).
A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee.
The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22, Division 12, Chapter 1 and Section CCR & H&S.
A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and explained. Exit interview conducted and report was reviewed with the Teacher White. |
