On July 23, 2024 at 9:30 am Licensing Program Analyst (LPA) A. Wallin conducted an unannounced complaint investigation for the purpose of delivering findings for the above allegation. LPA met with director, Linda White, and advised the of purpose of the visit. During inspection, LPA observed 28 children and three staff present. LPA observed proper care and supervision and ratio. During inspection, LPA conducted additional interview with licensee. Throughout inspection, LPA conducted interviews with eight out of 15 children in the Butterfly class and three staff. Note: (child #4 was not qualified to complete an interview).

After conducting interviews with multiple witnesses, the following conclusion has been reached: Substantiated - A finding that a complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. The facility was cited a Type A deficiency according to California Code of Regulations Title 22 (See LIC 9099D report for deficiency). A plan of correction was discussed and provided. Licensee is to post notice of Site Visit for 30 Days, failure to do so will result in $100 immediate civil penalty. This report must be copied and given to all parents and to the parents of any child enrolling within the next 12 months. A copy of LIC 9224 must be signed and retained in the file. During exit interview, director stated she was not comfortable signing report. No signtaure obtained during visit. An exit interview and a copy of this report along with Appeal Rights were explained and provided to licensee.

medical attention after obtaining a head injury. Based on interviews conducted, the licensee failed to ensure that children in daycare are receive safe, healthful, and comfortable accommodations. This poses an immediate health and safety risk to children in care.
