25 | On February 11, 2025, Licensing Program Analysts (LPA) Tyler Reyes and Jeanette Estrada conducted an unannounced Annual Required Inspection at the above facility. LPAs met with Director, Linda White, explained the reason for the visit and was guided on tour.
LPA was only able to work on the following domains:
Physical Plant:.The following was observed during the tour of the outside of the facility: equipment was inspected for age appropriateness and good repair. LPA observed outside an unlocked door accessible to children, with a water heater, paints, and gardening hedgers stored inside. A broken red metal (3) steering wheel play set was observed. 1 of 3 steering wheels was broken exposing a metal sharp on both ends. The Last Emergency Disaster Drill 1/30/25
Staff Records: LPA observed from record review of the LIC 500 and staff #1 (S1) file they are not cleared/associated. Review of S2's file they were not associated to facility 197416403.
Due to time constraints, LPA will return at a later date to complete remaining CARE Tool domains. Exit interview conducted with Director, Linda White and a copy of this report was provided. Appeal rights were discussed and distributed with a civil penalty in the amount of $1000.00. A copy of this licensing report dated 2/11/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
