Licensing Program Manager (LPM) Rita Ramos and Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced annual inspection to the above facility on 04/28/2023. LPM and LPA arrived at the facility at 09:40AM. LPM and LPA initially met with Eva Lopez, Principal and Celia Sosa, Assistant Principal who guided LPM and LPA on a tour of Room 2. LPA provided a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 1 classroom. Facility operation hours are Monday to Friday from 8:00 AM to 2:30 PM. Facility has two sessions: The AM session is from 8:00AM – 11:00AM and the PM session is from 11:30AM – 2:30PM.

All areas identified on this report were inspected. Upon arrival, there was 1 staff present with 11 preschool children. The facility was observed not to be within ratio. Per Staff #1, they are the only staff available at the time. Per Principal, they have been unable to fill a position to assist Staff #1. LPM and LPA advised Staff #1 and Principal that the ratio of 1 teacher for 8 children should be met at all times.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have a space with a hook to hang personal belongings outside the classroom. LPA observed children’s personal belongs touching as they were hanging on the hooks. LPA advised Principal that each child should have a personal storage area such as cubbies in which personal belongings shall not come in contact with other children's belongings.

Age-appropriate sinks and toilets were inspected for availability and good repair in the restroom. When inspecting the restroom, LPM and LPA observed that children need to walk through a storage room to get to the restrooms next to the classroom. LPA advised Principal that the storage area will need to be clean and cleared. LPA also addressed concerns of supervision while children go to the restroom since children should be supervised at all times. On 09/27/10 the Department provided a waiver approval for the use of shared restrooms noting that supervision will be provided at all times including when children are using the restrooms. Due to Staff #1 being observed singularly caring for 11 children, this requirement is not being met. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be accessible to children. Two bottles of Clorox disinfecting sprays were observed to be stored in low and unlocked cabinet under the sink in the classroom (pictures were taken). LPM and LPA also observed 3 insect traps under the sink. Per Principal, the traps are there for precautionary purposes since they are not aware of any insect or rodent concerns in the classroom. Disaster drill log was available, last drill was conducted on 04/26/2023. Carbon monoxide detectors were not observed. LPA did not observe any menus posted. The facility provides breakfast to children in AM session and lunch to the children in the PM session. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste were in good repair.

Outdoor playground equipment was observed and inspected. When inspecting the tricycles, LPM and LPA observed that one of the tricycles has a front tire that is torn (pictures were taken). LPA observed a child sized kitchen and bench with dilapidated/rotted wood (pictures were taken). The surface of the outdoor activity space is maintained in a safe condition. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. ---Page 2 of 4

There is adequate shade in the play yard. LPA reminded Principal to ensure that they are posting and complying with the outdoor waiver on file.

There are no firearms stored on the premises. There are no pools or bodies of water at the facility. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. LPA observed only 9 of the 11 children present signed in.
Children’s Records were reviewed. Inspection of required forms was made and documented on the LIC 857.

LPA also reviewed staff record's. The review of staff records was documented on the LIC 859. There was no proof of current AB 1207 Mandated Reporter Training certificate on file. There was no proof against TB, measles, pertussis, and influenza. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. First Aid supplies were observed in Room 2. According to Nurse Office staff, no medication is administered at this time. The facility was observed to be equipped with an isolation area for any child who becomes ill, and it is located in the nurse’s office.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.
