NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On March 19, 2026, Licensing Program Analyst (LPA’S) Calloway and Avalos made an unannounced inspection to the above facility for the purpose of conducting an Annual Random inspection. LPA’S met with Erin Johnson Facility Director as the representative who granted access. LPA’S and representative toured the facility inside and outside. LPA’S observed five infants in active play and one napping and three staff. The facility is open from 6:30 am to 6:30 pm. Monday-Friday. All employees 18 years or older that are working or volunteering have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Central Index Clearance and are associated with the facility. Physical Plant: The required postings were present on a visible parent wall near the front entrance. The sign in/out sheets were digital on the Brightwheel APP. There are two clean and safe classrooms, the telephone service, heating, ventilation, and lighting are adequate. The staff/ child ratios were observed to be compliant; The daily lesson plan were posted. There are cubbies for children's belongings that are labeled with their names in each classroom. The furniture, books, equipment, toys, and materials were appropriately aged and in good condition. There were activity rugs inside the classroom for play Representative stated the rugs are vacuumed daily and cleaned once a month. The drinking water is available inside the classrooms in the form of water pitcher and labeled water cups or disposable cups. Representative stated the cups are taken home daily. The napping equipment is mats and cribs and there was enough for all children. Representative stated the bedding is stored separately and washed weekly. LPA’Ss observed three high chairs and swing that were in good condition. The first aid kit was in the front office and the Emergency Survival Kit kept outside on the playground and there is water and food inside the kitchen. The trash cans were observed with secure lids and the temperature inside the room was comfortable.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Representative stated the fire extinguishers, smoke detectors and carbon monoxide detectors throughout the day care center are all operable and were checked during the last fire inspection. Inside the classroom there is a diaper changing table, and a handwashing sink nearby that is functioning properly with soap and paper towels available. LPA’S did not observe any accessible hazards. The staff restrooms are in the staff lounge outside of the classroom. There is a refrigerator with the bottles labeled for each child. The chemicals and cleaning supplies are kept in the custodial closet outside of the classroom.
Outside: Outdoor play equipment was inspected for safety, cushioning material, good repair and is age appropriate. LPA’S observed three small plastic play structures one swing set securely anchored in the ground with a canopy for shade and a rubber mat and grass as a cushioned surface. There is concrete and grass area for active play. The playground was observed to be free of debris. Drinking water is available in the form of a portable water pitcher and disposable and labeled water cups. Representative stated there are no bodies of water on the premises; LPA’S did not observe any. There were bike cars and small plastic rocking horses. There was an animal viewing pen that was empty. The fencing is wrought iron observed locked and is secure around the entire playground.
Other Review: Children and staff files were reviewed. Current Pediatric CPR/First Aid (expires: 8/2027). Representative stated transportation is not provided. Disaster Drills: Fire Drill was conducted: 1/6/2026 and the Earthquake drill was conducted: 2/7/2026. Representative stated the last facility fire inspection was conducted: 2/13/2026. The child/parent roster was complete with all required information. Representative stated the children are inspected for illnesses as they arrive. There is a separate isolation area for ill children in the site supervisor's office until the parent can pick up the child. Representative stated, the medications are stored in the secondary office and staff are aware prescription medication shall be administered in accordance with the label directions as prescribed by the child's physician. For each prescription and non-prescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child and inform their representative when medication was given. Each container shall have an unaltered label. LIC 125 form was discussed with the representative.

The following was discussed with the Representative:
Access the CCLD Licensing website at www.ccld.ca.gov. to obtain information about the most recent regulatory changes and the Quarterly Updates. Report any director or staff changes within ten (10) days. All representatives are responsible for knowing the regulations when providing care. All employees should know their role and responsibilities of being a Mandated Reporter. Training is renewed every two years at: mandatedreporterca.com. All adults 18 years and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a Criminal Record Clearance or Exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-
CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be
submitted to the Department. The following information regarding ADA was provided: US

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers that were constructed before January 1, 2010, to test their water (used for
drinking and food for lead contamination before January 1, 2023, and then every 5 years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA’S discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe sleep; as an additional resource. LPA’S also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. State law prohibits baby walkers, bouncy seats, exersaucers, bassinettes or any other items that fall into that category. All infants are sleeping on their backs unless specified with LIC 9227 form and no blankets, toys, or pacifiers attached. Conduct 15-minute nap monitoring and a napping log is kept daily for each infant.

Facility representative was informed of the MyChildCarePlan.org website, a
consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
