| On 09/23/2024 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes.
LPA Laureano met with Trisha Marlisa Assistant Director and toured the facility both indoors and outdoors. LPA observed 35 children and 5 staff members, including assistant director providing care and supervision.
LPA confirmed all adults have a valid criminal record clearance. LPA Laureano is following up on a self reporting incident that occurred at facility on approximately 9/16/2024. Incident report state that Child 1 shared with parent that Staff 1 hit a child. LPA reviewed incident report submitted on 9/18/2024 to the El Segundo Child Care Regional office.
Investigative interviews were initiated with staff and children and documents were reviewed. Based on information received today, further investigation is needed.
No deficiencies were cited during today’s inspection in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1 and California Health and Safety Code.
Upon on receipt of this report, the Licensee shall post the Notice of Site Visit.
The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
An exit interview was conducted, and report was reviewed with Assistant Director Trisha Marlisa. A copy of this report and appeal rights were discussed and left with the Licensee, whose signature on this form confirm receipt of these documents.
