On 11/18/24 at 9:10 AM Licensing Program Analysts (LPAs) A. Carter and T. Tran conducted an Unannounced Complaint Inspection for the purpose of delivering findings for the above allegations. Upon arrival, LPAs met with Interim Site Supervisor, Douglas Franco.
During the investigation LPA made observations, conducted interviews, and obtained facility records. Information gathered through interviews with staff and others confirmed sometime in the month of August 2024, the facility had two cases of head lice within the same family. Therefore, the allegation of facility failed to prevent the transmission of head lice did not happen because no other children were confirmed with head lice. During interviews, S4 and S5 indicated that C1 was fully potty trained upon transition into their class therefore the allegation of facility left a child in soiled diaper is not credible. The alleged parent did not provide the time or date when the incident occurred. Although the allegations may have happened or is valid there is not a prepondernace of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations were unsubstantiated.
Exit interview was conducted with Interim Site Supervisor, Douglas Franco.
