25 | On 12/04/2024 Licensing Program Analyst (LPA) Lisa Clayton arrived at the Bright Horizons Ocean Park Preschool unannounced, to conduct a Case Management – Incident inspection. LPA Clayton was greeted by Director Nara Keheyan. LPA Clayton observed 52 children in care being supervised and cared for by 8 fingerprint cleared staff.
INCIDENT DETAIL: On 11/18/2024, Jordan was going up the steps of the play structure and missed a step, his leg slipped, he fell forward and hit is head on the steps. He sustained a cut on the right side of his head on his right eyebrow. Teacher Mishalay called me and I went with a towel and an ice pack. We brought him inside and we were able to stop the bleeding. We called mom and she arrived within 15 minutes. Teacher Mishalay went with mom and Jordan to the urgent care where he got 5-6 stitches. He did not come back to school after seeing the doctor.
Incident update: Jordan returned to school on 11/19/2024 with no restrictions or physician instructions. Staff decided to keep him off of the play structure until the stitches healed.
During today’s visit, LPA Clayton observed Jordan smiling and talking with his classmates during lunch. LPA Clayton observed a small scar on his right eyebrow.
Based on the information obtained, interviews conducted and LPA Clayton's observation of the play structure, Jordan slipped while using the age-appropriate structure as it was intended, and there were no objects on the play structure where the child fell. In addition, the facility had appropriate children/staff ratios during the incident, and staff reported the unusual incident in a timely manner; therefore, no Title 22 violations have occurred, and no deficiencies cited.
Exit interview conducted and report was reviewed with Director Nara Keheyan.
LPA Clayton posted Notice of Site visit which to the remain posted for 30 days.
