25 | On 07/31/2024 at 2:35 PM Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 07/15/2024. Upon arrival, LPAs met with Facility Representative (FR), Patricia Ojeda, Principal. Census was taken.
On 07/15/2024, an incident was reported to The Department in regards to a child on child incident at the facility. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements.
During this inspection LPAs interviewed FR, who reported the incident. FR states that they conducted an internal investigation which included interviewing staff, parents, and children involved. FR states that children are in separate classrooms and have limited interaction through out the day, the only times children are together is during breakfast and nap time where there is always full supervision. FR states staff confirm that they have not observed incident and that children do not interact together. FR interviewed children and stated they did not provide any disclosures in regards to incident. As a precautionary measure, FR has conducted a training with Staff on 07/16/2024 on the topic of "Effective Supervision of Young Children". LPAs obtained and reviewed documents provided to Staff and observed that seven staff signed in for attendance to the meetings (Part I and Part II). FR states that children's Authorized Representatives were notified of follow ups conducted and there have been no additional concerns expressed moving forward. Based on the information obtained LPAs determined there was adequate supervision and follow up provided.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 07/31/2024.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Patricia Ojeda.
