25 | On 09/12/2024 at 12:00 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 09/03/2024. Upon arrival, LPA was granted entry to facility and announced purpose of inspection and met with Facility Representative (FR) Patricia Ojeda, Principal. LPA singularly toured facility. Census was taken. LPA observed children transitioning to nap time at time of inspection.
On 09/03/2024, an incident was reported to The Department where child's Authorized Representative alleged that her child's personal rights were violated at the facility by staff member. The facility reported this incident to The Department within the required 24 hours and submitted a written report via email.
During this inspection LPA interviewed FR, who reported the incident to Licensing, and she stated that she followed LAUSD protocols in calling Law Enforcement Agency (LAPD), submitted SCAR report, and informed LAUSD Operations. LAPD notified FR to handle Administratively and no investigation was conducted by them. FR states she has conducted investigation which involves interviewing staff and children. Per FR, interviews did not provide corroborating information. FR states she was not able to obtain any additional information from alleged victim due to child's limited verbal abilities. FR states child no longer attends and has been disnerolled but transferring to another LAUSD facility closer to home. FR states that at this time, the investigation is near completion, pending further discussion with Operations Manager on next steps. FR states due to this incident, she has conducted additional trainings with staff regarding Personal Rights for children and Code of Ethics. LPA reviewed sign in forms for training at time of inspection. Based on the information obtained, there was no violation of personal rights at the facility at this time though there is pending information. LPA requested FR to provide update to Department should one become available.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 09/12/2024.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the FR, Patricia Ojeda, Principal.
