On 3/11/2025 at 9:45 AM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 12/11/2024 and 1/16/2025. Upon arrival, LPA met with Facility Representative (FR), Patricia Ojeda, Principal. Census was taken.

On 12/11/24 and 1/16/25, an incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements.

During this inspection LPA interviewed FR, who reported the incident to Licensing, and she stated that she followed LAUSD protocols in calling Law Enforcement Agency (LAPD), submitted SCAR report, and informed LAUSD Operations. LAPD notified FR to handle Administratively and no investigation was conducted by them. FR states she has conducted investigation which involves interviewing staff and children. Per FR, interviews did not provide corroborating information for 12/11/24, or 1/16/25. FR states she was able to confirm with alleged victims. FR states children both continue to attend care. FR states that at this time, the investigation is complete and is pending decision from operations. FR states due to this incident, she has conducted additional training with staff regarding Personal Rights for children. LPA reviewed sign in forms for training at time of inspection. Based on the information obtained, there was no violation of personal rights at the facility at this time though there is pending information. LPA requested FR to provide update to Department should one become available.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 03/11/2025.

A notice of site visit was given and must remain posted for 30 days.
