On Friday, September 13, 2024, at 2:30 PM, Licensing Program Analyst (LPA) Mayra Rivera, conducted a Case Management at the facility to deliver an Immediate Exclusion from any facilities notice to staff Iresha Kumar Weerakkody-Gedara.

LPA met with director Rushani Thomas-Wise. During this inspection, LPA entered classrooms Hazel, Willow, Aspen, Maple, Secoia, Redwood and Cypress. LPA observed a total of 91 children and 16 staff present providing care and supervision. LPA observed the facility to be within ratio. LPA Rivera observed Iresha Kumar Weerakkody-Gedara present.

LPA informed Director Rushani Thomas-Wise, that Iresha Kumar Weerakkody-Gedara is excluded from Montessori of Stevenson Ranch # 197419166 and any other licensed facilities by the California Department of Social Services (CDSS) as a result of investigation by CDSS of the allegation of Conduct Inimical and Personal Rights. The Department has determined that continued presence constitutes threat to the health and safety of children in care. This action is final unless otherwise notified by the Department. Appeal process and instruction was reviewed and provided to Iresha Kumar Weerakkody-Gedara and director,

The Confirmation of Removal licensing form LIC300E was signed during this inspection. LPA Rivera and director Rushani Thomas-Wise escorted Iresha Kumar Weerakkody-Gedara outside the facility during this inspection.

A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report and appeal rights were reviewed with director Rushani Thomas-Wise.
