On 08/16/23, Licensing Program Analysts (LPAs) Justeene Tamayo and Evelyn Garcia conducted a complaint inspection for the purpose of investigating the above complaint allegation. Upon arrival to the facility, the LPAs were greeted by Director, Angie Serrano and the nature of the visit was discussed. During this initial investigation, LPAs interviewed staff and children present. LPAs observed 61 children and 10 staff members.

During interviews, it was revealed child #1 was left unattended for about 5-10 minutes in the playground area. The Department also received a Unusual Incident Report dated 06/19/23 of this incident occurring. Based on the information obtained, the above allegation is deemed Substantiated. A finding of substantiated means that allegation is valid. Facility has been cited a Type B citation Responsibility for Providing Care and Supervision 101229(a)(1) Please see LIC-9099D for more information.
