On 1/27/2022 Licensing Program Analyst (LPA) Ortega met with Facility Director to conduct an unannounced case management inspection. The purpose of the case management was to address an incident that occurred on January 27, 2023. The incident pertained to a violation of children's personal rights, this incident involved child #1 (C1) and Staff #1(S1). LPA Observed and heard (S1) inappropriately yell, shout and handle (C1) while other children were present in care. (S1) was observed and heard utilizing a loud, stern voice when speaking with the child in care (C1).

During LPA's classroom observation dated 1/27/2023, Staff #2(S2) was observed inappropriately redirecting children while in care. LPA observed (S2) interact with children (C1), (C2), (C3), (C4) in the classroom treating children without dignity and utilizing shame, humiliation and threat.

Based LPA's observation and interviews conducted during today's inspection, the facility is being cited Type B citation for Personal Rights violation. Please see LIC 9099D.

An exit interview was conducted, and a copy of this report was provided to the facility Director on this date, along with a copy of the appeal rights and Notice of Site.



Notice of site visit must remain visible to parents and posted for 30 days.

LPA's observation witnessing (S1) and (S2) did not accord the child/children dignity when (S1) yelled at the (C1)while in care and while other children present. (S2) inappropriately interacted with (C1), (C2), (C3) and (C4). This poses a potential risk to the health and safety of the children in care.

LPA's observation: (S1) and (S2) humiliated/shamed, punished the child/ren in care while other children were present. (S1) yelled "shut your mouth" in close proximity addressing (C1). (S2) stated to multiple children "are you a baby" utilizing shame and used statements such as IF you..I am going to... This poses a potential risk to the health and safety of the children in care.
