Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced case management inspection. LPA met with Anvar Jourabchi, Assistant Administrator & Akram Guivatchian Administrator who guided analyst on tour of the facility. The facility operates Monday through Friday from 7:30am to 2:30pm. The entrance to the facility is locked and parents and visitors enter by using using a call/buzzer system. There is a school age program on the premises (#197419377) that runs from 2:30pm to 4:30pm. There is also an Elementary School on the premises that runs from 7:30am to 2:30pm.

LPA come to conduct an annual inspection for School- Age program 197419377 when LPA observed at 12:00pm that 4 Elementary School students were in the preschool playground. At 12:05pm, LPA observed a Elementary School Student walk into the Preschool classroom to obtain there belongings. LPA reminded staff that co- mingling is not allowed. Consult was provided. Type A was cited.

Based on LPAs the facility is being cited under the California Code of Regulations, (Title 22, Division 12, Chapter number 1) are being cited on the attached LIC 809 D.



The Administrator was provided with LIC 9224 Parent Acknowledgement of Report Received.
A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC 9224) shall be in each child’s file, acknowledging receipt.
An exit interview was conducted and report was reviewed with the Administrator. Appeal rights were discussed and proved to the Administrator.

Per Administrator, the facility will review Title 22 regulation 101161(a) and will ensure they adhere to title 22 section 101161(a). Administrator stated that she will submit a wavier to the department to use the preschool playground while preschoolers are having lunch.

co-mingling at 12:00pm, LPA observed 4 elementary children in the preschool playground. At 12:05pm LPA obderved a elementary child going into the preschool classroom to obtain there belongings. This is a immediate risk to the health and safety of the children in care.
