25 | On 3/11/2025, Licensing Program Analysts (LPA’S) Doris Whitmore and Patsy Plancarte conducted a case management inspection to follow up on an Unusual Incident, reported to the Department on 03/04/2026 & 03/06/2026. LPA’S was greeted by the Teacher Lead Silvia De la Rosa and Regional Site Director La Shaye Davis LPA toured the facility and took a census of the children. LPA observed a total of 40 children in care with 12 staff members.
Description of the incidents: RP states at 3:30pm a call was made to DCFS since all phone numbers on the contact list were exhausted, and child ( C1) had not been picked up. The center had already been closed for an hour, and the child’s father showed up at approximately 4pm intoxicated, and center staff made a decision to not release the child to the father. Child’s mother showed up at 5:05pm to pick up the child.
On 3/5/26 around 2:30 pm, Parent (Father) of C1 showed up to facility to pick up C1 but, teacher stated he smelled intoxicated and would not release C1 to the father. Father became aggressive and was told to leave. Police and social worker were called but never showed up. Parent (Mother) came to pick up C1 around 4PM.
During this inspection, LPA’S toured the facility, interviewed staff and obtained the following documents, Policy Late Pick up a child, Sign in & Out Sheets, Personnel Record, Emergency Information,Service Contract Summary& Acknowledgement of Receipt Parent Handbook.
Based on the information provided and interviews conducted further investigation is required.
An exit interview was conducted, a conducted, a copy of this report and notice of site visit were provided.
