25 | On April 4, 2025, Licensing Program Analyst (LPA) Annelise Villa met Licensee Kushani Garusinghe who guided analyst on a tour of the facility for the random annual inspection. Upon arrival, LPA 8 children in care and Licensee and 2 assistants caring for them. Family members living in the home are Licensee, Licensee’s husband, licensee’s mother, 1 adult child, and 1 minor child. Hours of operation are Monday-Friday, 7:00am-5:30pm. Incidental Medical Services (IMS) policy was discussed.
Physical Plant: This two-story, 5-bed, 3-bath with living room, family room, dining room, kitchen, laundry room, and front/back yards. Main care is provided in the living room and dining room. Children use the bathroom #1 on the right of the home. Children have access to the main care areas, bathroom #1, and back yard. Off limit areas include bedroom #1 (downstairs), all upstairs areas including bedrooms 2-5 and bathrooms 2-3. Stairs were observed to be barricaded by a safety gate, which prevents children from accessing the off limits upstairs areas. The attached garage was observed to be locked during the time of this inspection. The garage is used for storage only and is garage is off limits to children. The home was inspected inside and out for safety, clean and orderly, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds, and medicines. Hazardous items (sharp knives are located in an upper kitchen cabinet, cleaning compounds are stored in garage which is off limits, and medications are stored upstairs and inaccessible to children in care.
Safe and age-appropriate toys, play equipment and materials were observed. LPA tested the smoke detector and carbon monoxide detector were observed both to be in operable condition. Fire extinguisher (2A10BC) was found to be in operable condition located in the kitchen. Electrical outlets were inaccessible. No recalled and or prohibited toys or play equipment were observed on the premises. There is a designated area for ill children as necessary.
