On 7/15/24 at 9:30 am Licensing Program Analyst (LPA), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with licensee Sharoniece Swain and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. Not all center staff that was present during today’s inspection had fingerprint clearance or associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate.

The hours of operation are 7:30 am – 6:30 pm, Monday - Friday. Facility is currently licensed for a capacity of 25. At the initial start of the site visit, there is a total census of 8 children and a total census of 2 teachers present. The facility is made up of 2 classrooms: Room 1: had 8 children and 2 staff, Classroom 2 is not being used currently. Class room 2 is the toddler classroom and there are no toddlers enrolled at present time, the room was observed to be not in use and is used for storage of napping cots and mats. Classroom 1 was equipped with cubbies with toys and areas for quiet time, toy kitchen area for dramatic play. Children were observed creating crafts at the child size table and chairs. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan (not posted), LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, menu and activity schedule (not posted).

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected.

LPA observed children's bathroom to be equipped with a toddler size toilet and sink and cubbies for a change of clothes. The Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. In the care area, LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed first aid kit kept in the classroom. First Aid supplies were inventoried and were brought into compliance with regulation during the visit.

Breakfast, snack and lunch menus were reviewed at time of inspection. Facility provides breakfast, lunch, and snacks. Food and snack items were observed to be properly stored. Food preparation area was toured and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via a filtered water dispenser. Carbon monoxide detector was observed but not in service and smoke detectors are present in the facility. LPs confirmed detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 9/1/2024.



At 9:55 AM, LPA observed outdoor play area which consists of multiple child size tables under a large shade structure. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPA observed uneven ground and rough wood railings with exposed nails that is in the process of being repaired. LPA observed bikes, bins with outdoor toys, balls, and other age-appropriate toys around the play yard. Required shade and fencing were inspected, fencing was observed to be a hazard at time of inspection due to children are tall enough to reach the wire. LPA advised Licensee that the children need to be always within the direct care and visual supervision by the teachers. Per Licensee, the children have access to drinking water via water bottles. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPA reviewed with licensee Sheronice Swain the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPAs observed current CPR and First Aid Certification for center staff. CPR card expired on 7/2023 for Staff Member. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Last fire drill conducted 5/30/24. Role and responsibilities of being a Mandated Reporter were reviewed.

The following were discussed with the Licensee: The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assists in providing care.

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


Based on this information the following deficiencies on the LIC 809 D are being cited today 7/15/2024.

LPA Claudia Kam informed facility representative Sheroniece Swain that this report dated 7/15/2024 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care regarding uncleared adult.


In addition, LPA Claudia Kam informed the facility representative to provide a copy of this licensing report dated 7/15/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days.

Administration of Child Day Care Licensing
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.
