25 | Licensing Program Analyst (LPA) Roberto Luque Avila conducted an unannounced POC case management visit to the above facility on 12/18/2024. LPA arrived at the facility at 1:45PM, identified self and met with Sheila Apostol, Licensee who guided analyst on a tour of the facility. LPA observed 6 children upon arrival. The purpose of this visit is to follow up on the technical advisory and deficiency cited given on 12/17/2024.
On 12/17/2024 LPA observed a child 1 inside a bassinet directly being supervised by staff 2 in the kids room. The bassinet poses a potential fall risk to the child in care because the infant is able to stand inside the bassinet. The bassinet is not appropriate for the child to be placed in because the warning label stated: "Fall Hazard- To help prevent falls, do not use this product when the infant begins to push up on their hands and knees or has reached 20 lb (9kg), whichever comes first." Licensee removed the child from the bassinet and made it inaccessible to children in care. A type B deficiency was cited for the bassinet being a potential fall risk to the child.
During the inspection dated 12/17/2024 LPA also observe a stationary walker in the home during the inspection. This item is not allowed in a child care home, however there was no child in the walker and was not accessible to children. A technical advisory was given.
On 12/18/2024 LPA did not observe the stationary walker or bassinet in the home. Thus, clearing the deficiency cited on 12/17/2024.
A notice of site visit was posted and must remain posted for 30 days.
Exit interview was conducted, appeal rights and report were provided to the licensee Sheila Apostol.
