**This is an amended report**
On 8/7/2025, at 9:00AM Licensing Program Analysts (LPA) Devon Carus, and Brittany Lovest conducted a complaint initial visit regarding the above-mentioned allegations. Upon arrival LPAs met with Sun Ko, Director. LPAs explained the purpose of the inspection. LPAs observed 19 children in care supervised by 4 Staff.

During today's inspection LPAs observed 13 children with Staff #2 and Staff #3 on the playground. While staff were present on the playground, they were not always in visual range of the children which poses an immediate health, safety or care and supervision risk to persons in care.

During today's inspection LPAs observed 3 loose water hoses, and two broken toy cars in the outdoor play yard while children were present, which poses/posed a potential health, safety or personal rights risk to persons in care. Director was informed to move the items to an area inaccessible to children in care.

During today's inspection LPAs observed 3 expired food products in designated Preschool refrigerator, which poses/posed an potential health, safety or personal rights risk to persons in care. Director informed LPAs that this food was intended to be used for children, and director was informed that all food must be fresh and any food items expired must be discarded immediately. LPAs observed director discarding all expired foods from refrigerator during visit.

Based on the evidence & information obtained during the investigation, which included interviews with relevant parties, & observations, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated.

1 Type-A Violation, and 2 Type-B violations will be cited today.

Licensee must comply with AB 633 as follows: Upon receipt by the licensee, licensee is to provide to parents/guardians the following: Copies of any licensing reports that document a Type A citation - this includes facility visits and substantiated complaint investigations; copies of licensing documents pertaining to a conference conducted by a local licensing agency management representative and the licensee of this child care center in which issues of noncompliance are discussed and/or copies of a summary of an accusation indicating the Department's intent to revoke the facility's license.

Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of children currently enrolled and any newly enrolled child at the facility for the next 12 months. LIC 9224 must have the signature of child's authorized representative and kept in the child's file.

Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
