13 | On October 20, 2021, Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced complaint subsequent inspection on the above allegations. LPA disclosed the purpose of inspection and was permitted entry by Director who guided LPA on tour of the facility. LPA counted 27 children with 7 staff present.
During inspection, LPA interviewed Director, Staff and obtained facility records. On both inspection dates, LPA observed facility to be within ratio, file records were reviewed and staff demonstrated qualifications. Time sheets and child attendance records were also observed and found to be incompliance. Supervision is being conducted by qualified staff thrpoughout the day.
Based on evidence obtained and interviews conducted, the above allegation is deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations did or did not occur. An exit interview was conducted, a copy of this report, Appeal Rights, and a notice of site visit were provided to the Director, Shirley Johnson. |
