13 | On October 20, 2021, Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced complaint subsequent inspection on the above allegation. LPA disclosed the purpose of inspection and was permitted entry by Director who guided LPA on tour of the facility. LPA counted 27 children with 7 staff present.
During this investigation, LPA interviewed Director, Staff, children and obtained facility records. Time sheets and child attendance records were also observed and found to be incompliance. LPA observed staff rotating and communitcating during breaks and lunch, supervision was observed at all times on both inspection dates. Interviews did not disclose children being left unattended while in care.
Based on evidence obtained and interviews conducted, the above allegation is deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations did or did not occur. An exit interview was conducted, a copy of this report, Appeal Rights, and a notice of site visit were provided to the Director Shirley Johnson. |
