25 | On 02/18/2025 Licensing Program Analysts (LPA) Cristina Castellanos conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 02/03/2025. Upon arrival, LPA met with Lead Teacher/Coordinator Chilien Hsu and explained the purpose of the visit. LPA then toured the facility both indoors and outdoors. Licensee Fong Hsu arrived at approximately 01:40pm.
There were 26 children present at the facility, 2 Teachers and 4 Teacher Assistant.
According to the UIR, on 01/31/2025 during the outdoor playtime the child named on the report fell forward and hit his face on the concrete area.
LPA requested the following documents: Parent handbook, children's roster, staff roster, personnel files and the pre-school daily activity schedule. Furthermore, LPA took pictures of the outdoor play areas and initiated staff interviews.
During today’s inspection, LPA observed all three classrooms: Big, Middle, and Small to be out of ratio. From 12:15pm to 12:45pm LPA Castellanos observed 8 children with one staff member (S4), identified as a teacher assistant by Lead Teacher C. Hsu and staff member herself. Then at 12:35pm LPA Castellanos enter the Big and Middle classrooms and observed 18 children of whom were awake with just one staff member S3. Around 12:45pm staff member S2 then enter the classroom.
Based on observation, the facility was operating out of ratio - 101216.3 Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below, which poses an immediate health, safety or personal rights risk to persons in care.
