On March 3, 2026, Licensing Program Analyst (LPA), V. Wheatley conducted an unannounced inspection regarding the above allegations and met with licensee Saroja Basnet. LPA observed 12 children on the premises napping of which three are infants. LPA observed two assistants on the premises and interviewed the assistant that was absent during last inspection.

On February 25, 2026, LPA V. Wheatley conducted a complaint investigation. LPA met with licensee Saroja Basnet and explained the purpose of the inspection. LPA toured the home inside and outside. LPA observed 12 children on the premises of which five infants are in care. During this visit, LPA interviewed three children. LPA informed licensee that another visit will be conducted to interview an employee on another day.

On December 12, 2025, LPA V. Wheatley conducted a complaint investigation. LPA met with licensee Saroja Basnet and explained the purpose of the visit. LPA toured the home inside and outside. LPA observed 9 children on the premises. Eight children were outside and one infant child inside asleep. LPA observed a total of four infants in care. During this visit, LPA interviewed two staff members. LPA reviewed children's files. LPA received a copy of the children's roster. Licensee reported the incident to the Departmrent and submitted a LIC 624 and declaration. See Page 2

Based on investigation that included interviews with licensee, staff, children information revealed that on more than one occasion the licensee utilized inappropriate discipline to get children to comply. Most recently the utilized a pointer stick to discipline a child. The preponderance of evidence standard has been met; therefore, allegations of personal rights is found to be Substantiated.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit (LIC 9213) and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).

The Acknowledgement of Receipt (LIC 9224) form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224).

Exit interview was conducted. The report was read and provided to the licensee. The Notice of Site Visit was provided. A copy of appeal rights was provided to licensee.

Licensee has agreed to submit a step-by-step plan on appropriate discipline. (Personal Rights regulation was provided to licensee) due on date 3/4/26 to CCL via email. Licensee shall attend the in-person Family Child Care Home Orientation on date April 7, 2026, 9:00 am at Crystal Stairs. All assistants will complete the on-line orientation & submit certificates by date_April 8, 2026.

This requirement was not met as evidenced by: Based on investigation Licensee used of inappropriate discipline and a pointer stick. In addition, the other staff have used a pointer stick. This is a Type A violation. This is an immediate risk to the health and safety of children in care.
