13 | On 6/12/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced follow up complaint inspection for the purpose of delivering the findings for the above-mentioned allegations. Upon arrival, LPA was greeted and let into the facility by Director, Karen Beninati and facility representative, Vanessa Rodriguez to whom the reason for the inspection was announced. LPA toured the facility and observed 14 daycare children and 5 staff.
During the course of the investigation, LPA Ornelas made observations, obtained documentation in the form of children’s roster, LIC624 Unusual Incident Report, facility notices and conducted interviews in regard to the above allegations.
-Pertaining to the allegation that - Staff does not prevent out of hand, foot, and mouth outbreak.
According to Staff interviewed during the course of the investigation, parents were immediately notified via emai and via the school portal of the Hand, Foot, Mouth (HFM) case. Children were sent home, facility was deep cleaned and disinfected. Per staff, only 2 children were diagnosed with HFM. |
