On 6/12/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced follow up complaint inspection for the purpose of delivering the findings for the above-mentioned allegations. Upon arrival, LPA was greeted and let into the facility by Director, Karen Beninati and facility representative, Vanessa Rodriguez to whom the reason for the inspection was announced. LPA toured the facility and observed 14 daycare children and 5 staff.

During the course of the investigation, LPA Ornelas made observations, obtained documentation in the form of children’s roster, LIC624 Unusual Incident Report, facility notices and conducted interviews in regard to the above allegations.

-Pertaining to the allegation that - Staff does not prevent out of hand, foot, and mouth outbreak.
According to Staff interviewed during the course of the investigation, parents were immediately notified via emai and via the school portal of the Hand, Foot, Mouth (HFM) case. Children were sent home, facility was deep cleaned and disinfected. Per staff, only 2 children were diagnosed with HFM.

According to Parents interviewed during the course of the investigation, parents stated that a notice was sent about the HFM case. Per parents, they were notified in a timely manner. Based on interviews conducted with parents, LPA Ornelas was able to confirm two cases. LPA Ornelas observed a notice sent to parents regarding HFM.

-Pertaining to the allegation that - Staff does not prevent out of lice.
According to Staff interviewed during the course of the investigation, they had one child with lice and immediately sent out a notice informing parents and providing resources.
According to Parents interviewed during the course of the investigation, parents stated that they were notified in a timely manner and were provided resources. LPA Ornelas observed notice sent to parents.

-Pertaining to the allegation that - Staff not keeping facility clean.
According to Staff interviewed during the course of the investigation, staff stated that they clean regularly and extra cleaning was done when lice and HFM was taking place. Staff stated that they washed all soft equipment and sanitized every hard surface to stop the spread.
According to Parents interviewed during the course of the investigation, facility is clean and they have no concerns regarding the sanitation practices or the cleaning that took place during the time lice/HFM was present. LPA Ornelas observes teachers cleaning regularly upon visits that are conducted.

Based on the evidence as documented above, the allegations have been determined to be Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegation occurred.
A notice of site visit was given and must remain posted for 30 days.
