32 | The facility was place on lock down until after 3:00 p.m. when LAPD cleared the facility. LAPD investigative report indicates “Unit suspects punctured glass”. Staff were not instructed to notify parents of the incident. The glass was replaced the following day.
The facility contacted the El Segundo Regional office on Wednesday, 08/10/22 at 4:21 pm and left a voicemail on this LPAs voicemail, notifying LPA of an Unusual Incident Report.
After considering available information, the facility failed to notify parents of the facility being placed on lock down due to a possible active shooter; Facility failed to notify the El Segundo Regional of the Unusual Incident within the next working day. Based on LPAs interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California code of Regulations, Title 22, Division 12 & Chapter, are being cited on the attached LIC9099D.
During today’s visit two Type B violation are being issued under Title 22 Regulations, Division 12, Chapter 1, of the California Code of Regulations: 101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...; and 101212 Reporting Requirements (f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
