On 6/4/24, at 4PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. During a cite visit conducted at the facility on 6/4/24, LPA toured the facility with the assistance of the Director / Marisol Nepomuceno at 1:30PM and observed 77 children with 6 staff present in the preschool program.

The facility is licensed for a maximum capacity of 70 children. The licensee is operating a child care center beyond the conditions and limitations specified on the license, including the capacity limitations. This poses an immediate health, safety or personal rights risk to persons in care.

The following deficiency was observed to be in violation of California code of Regulations, Title 22, Division 6 (refer to 809D).

Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report
documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive
days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this
report shall be provided to the parent/guardian of children currently enrolled by the next business day or
immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly
enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be
maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of
the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Director and Appeals Rights provided.

Licensee will review Title 22 Regulations Section 101161 on Limitations on Capacity and develop a written Plan of Correction (POC) to ensure compliance. Licensee will also submit a declaration stating that facility will no longer operate beyond the conditions and limitations specified on the license.
