25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to a self-reported incident that occurred at the facility. LPA arrived at the facility at 8:40AM and met with Marisol Nepomuceno, Director, who guided LPA on a tour of the facility. There were 49 children in care and 7 staff present upon arrival.
The incident was reported to the Department on 02/05/2025, via telephone. The facility did not reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Parent of Child #1 reported to the facility that Child#1 may have sustained an injury while in care.
During today's inspection, LPA conducted interviews with Director, Staff #1 through Staff #5 and Parent #1. LPA obtained written statements from staff for this incident. LPA also obtained a copy of the facility roster and reviewed records. At this time, additional information is needed to determined if their were deficiencies while child #1 was in care.
There were no deficiencies cited during today’s inspection.
The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
Exit interview was conducted and report was reviewed with Marisol Nepomuceno, Director. |
