25 | On July 3, 2024 at 1:16 pm, Licensing Program Analyst (LPA) A. Wallin and Licensing Program Manager (LPM) K. Chambers conducted an unannounced Case Management Annual Continuance Inspection for the infant center license. During today’s inspection, LPA met with director, Jamesetta Willis, who provided assistance and toured the facility. Days and hours of operation are Monday through Friday from 6:00 AM – 6:00 PM. This facility consists of four classrooms (one infant class, two preschool classrooms and one school-age classroom) and outdoor play area. During inspection, LPA observed 3 children and one staff present. Per director, four infant children enrolled at this time. LPA observed 10 preschool children and two staff present during inspection. Capacity and limitations as specified on the license are being maintained.
LPA advised director purpose of inspection to complete children and staff record review not completed during previous annual inspection conducted on 6/11/24. During inspection, LPA observed 3 children’s files. Per record review, LPA observed one infant missing completed LIC 9227 form. A technical violation citation issued. Per record review, LPA observed infant needs and services plan missing for all infants enrolled. A type B citation issued.
Per staff record review, LPA observed the following documentation maintained and updated: LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, LIC 503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Per staff record review, LPA observed S3 does not meet all the qualifications of Health and Safety code 1597.005 concerning teacher qualifications. During inspection, LPA reviewed with director Health and Safety code 1597.005, provided director with copy of regulations and provided consultation regarding teacher’s aide supervision. During visit, director immediately corrected by removing S3 from classroom and placing qualified preschool teacher with infant qualifications in classroom. During visit, LPA observed S3 under supervision of a qualified teacher. A technical violation citation issued. Per record review, LPA observed infant aide staff missing complete immunization record. A type B citation issued.
