25 | Licensing Program Analyst (LPA) Austin Estrada conducted an unannounced case management inspection at the above facility to follow up on two incidents that were reported to the Department. Upon arrival, LPA met with Teacher Elizabeth Hernandez who provided LPA a tour of the facility. LPA observed 18 children and three staff in classroom 2.
On 5/17/23, an unusual incident report was made to the department regarding an incident that occurred on 5/16/23 which involved a child who was touch in the “private parts” by a family member. This was brought to the attention to Staff #1 (S1) by the mother of child #1 (C1). The facility reported this incident to the Department within the required 24 hours. The facility also reported the incident to Child Protective Services. LPA conducted interviews with children and staff members during this inspection.
On 5/19/23, an unusual incident report was made to the department regarding an incident that occurred on 5/18/23 in which staff #2 (S2) observed a bruise on child #2 (C2) right thigh. The facility reported this incident to the Department within the required 24 hours. The facility also reported the incident to Child Protective Services. LPA conducted interviews with children and staff members during this inspection.
Based on interviews conducted, Per California Code of Regulations Title 22, Division 12, no deficiencies are be cited during today's visit as the facility adhered to the proper reporting requirements. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview was conducted and report was reviewed with Teacher Elizabeth Hernandez. Appeal rights were discussed and provided to Teacher Elizabeth Hernandez.
