On 05/12/2023 at 1:30 PM, Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Annual Required - 1 Year inspection to the above facility. LPA announced purpose of visit and initially met with Sandy McGlothan, Assistant Director, who granted entry to the facility. This is an infant program which consists of one classroom located to the rear of the building. A Preschool component (#197493731) is on the front unit of the property and a School-Age component (#197493729) is in middle room next to Infant Program Facility operation hours are Monday to Friday from 5:30 AM to 6:30 PM. LPA was provided a guided tour of the facility both indoors and outdoors. LPA observed there are separate entrances to each component.

All areas identified on this report were inspected. Upon arrival, there is a total census of three napping infants present and a total census of one teacher. LPA observed infants sleeping in infant rocking chairs in Activity Space. LPA advised S1 that per Title 22 Regulation requirements infants are to be moved to cribs once asleep. LPA observed infants moved at time of inspection, one continued sleep in a crib in designated sleep area during inspection. Classrooms were observed to ensure that infants are never left unattended and under the direct visual supervision of a staff person at all times.


Children's roster was reviewed and is current. Sign-in and out procedures were reviewed. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children present were signed in. Disaster drill log was available, last drill was conducted on 04/06/2023.

Furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. All materials and surfaces accessible to children are toxic free. Classroom was inspected to ensure that the floors have a surface that is safe and clean.

Children have their own cubby to store their belongings. Six wooden cribs were observed in a separate designated sleeping area of the room. Storage for infants’ belongings and napping equipment/cribs was inspected and meet all regulatory requirements. Per Staff, linens are provided by facility and washed by staff weekly. LPA observed wall unit changing tables available with cushioning in nap area and activity area both within arms reach of sink. LPA discussed diaper changing procedures with S2.

Telephone service, heating, lighting and ventilation were evaluated and are in operating condition. Per S2, the isolation area is located in isolation area of room that will be kept separate from other infants. LPA observed a wooden crib available in the isolation area for ill children. Parents are contacted immediately when children are determined to be ill. Availability of drinking water was reviewed, per S2 infants are provided water through their bottles which is taken outside. LPA observed first aid kit kept in cabinet and was inventoried for necessary supplies.



Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible. At 3:10 PM, LPA observed medicines stored in open plastic bin in cabinet in classroom. LPA did not observe child's name labeled on 3 of 4 medicines and no date on 4 of 4 medicines at time of inspection. Per S2, 3 of 4 medicines were for a child no longer attending facility, LPA observed S2 dispose of medicines at time of inspection. Facility has one or more functioning carbon monoxide and smoke detectors that meet statutory requirements.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. LPA did not observe bottles, dishes, and containers to be labeled with infants’ names consistently visible. LPA did observe names re-written on bottles during inspection, per S2 bottles are labeled but erase after multiple washes. LPA did not observe dates visible on all children's bottles and in refrigerated items. Formula and snacks are provided by parents and dates observed. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides snacks and meals for infants who eat table food (per needs and service plan). All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. All foods/beverages are stored in covered containers at 45˚ (F) or less.

LPA observed outdoor space is physically separate from other child care components and has separate entrance. LPA observed outdoor play equipment was to be in good condition, free of sharp, loose or pointed parts. LPA observed outdoor activity space surface not maintained at time of inspection. LPA observed turf grass covering matting and wood chips serving as cushioning material to be torn in various areas of outdoor play area. LPA observed wood chips and matting material to be exposed at time of inspection. The outdoor area has adequate shade.

Assistant Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Director states there are no weapons or firearms on the premises.

All individuals present have obtained a criminal record clearance or criminal record exemption. Staff records were reviewed for completeness and documented on LIC 859. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA observed S2 Pediatric First Aid/CPR to expire on 06/23/2023. LPA did not observe current completion of Mandated Reporter training AB1207 at time of inspection. Infant records were reviewed for completeness and documented on LIC 857. Required forms were discussed with Facility Representative at time of inspection.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Upon receipt of this report, the licensee shall post ANY licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care and any newly enrolled children for up to one year.

The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
