1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/27/2023 at 1:50 PM, an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Franchesca White. LPAs announced purpose of visit and initially met with S1, Preschool Teacher, who granted entry to facility. Facility is currently licensed for a capacity of 42. The program currently operates Monday thru Friday from 6:00 AM to 6:00 PM. LPAs were taken on a guided tour of the facility of both indoors and outdoors by Lindsey Escobar, (S3). Facility also has Infant Program (#197493730) and School Age Program (#197493729). At the initial start of the site visit, there is a total census of 13 children and a total census of four staff present (S1, S2, S3, and S4). LPAs observed 13 napping children upon arrival to facility with S1 present. The facility is made up of one classroom. LPAs observed cots laid out in room that does not hinder any walkways. Per Staff, sheets are provided by facility and washed weekly. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. LPAs observed wall A/C units through out and a wall heater with a barrier making it off limits to children in care at time of inspection. There were cubbies observed as storage for children's belongings. LPAs observed child size tables and chairs, wooden play kitchen, art materials, and other age appropriate toys free of loose and sharp parts and well maintained at time of inspection. Availability of drinking water was reviewed. LPAs observed water pitchers with paper cups available for drinking water in classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed two bathrooms available for children, one with one sink and one toilet and another with two sinks and two toilets, totaling three sinks and three toilets available. LPAs reminded staff that supervision is required in bathrooms including visual supervision at all times. Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPAs observed plastic play structure with slides and climbing equipment with cushioning material below to absorb fall all well maintained. LPAs observed benches for Report Continues - Page 1 of 3

seating under shade available for children and hula hoops, balls, and other age appropriate toys free of loose and sharp parts available for children. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Per S3, staff brings out water pitchers with paper cups for outside drinking water. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

A first aid kit is kept in classroom and LPAs inventoried for supplies. LPAs observed moke detectors are present in the facility and fire extinguishers have been serviced on 02/16/2023. LPAs did not observe an operable carbon monoxide detector at the facility at time of inspection. Per S2, facility was recently repainted and carbon monoxide detectors taken down and were not returned. Disaster drill log available and last drill conducted on 04/06/2023. Sick/isolation area is in area of Preschool Room away from other children in care. Hazardous items including poisonous cleaning compounds were stored inaccessible to children, LPAs observed these items stored in Staff bathroom which is off limits to children in care.

Snack/lunch menus were reviewed and observed posted in Kitchen. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Kitchen is located in rear of facility near Infant Program. Facility provides AM Snack, Lunch, and PM Snack and children eat in the classroom. LPAs observed children eating in classroom and food being brought from Kitchen to classroom by staff.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. LPAs observed all children present were signed in at time of inspection Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed and documents on LIC 857 and LIC 859. Criminal Record Clearances were reviewed for adults. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file. There is at least one staff member trained in CPR and Pediatric First Aid present during this inspection. CPR card expires on 11/2023 for S4. Per Staff, no children in care required medications at this time.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
