On 9/28/2021 at 9:45 AM Licensing Program Analyst (LPA) Angelica Ramirez arrived at Beach Babies #5-Preschool for the purpose of delivering complaint findings for the complaint received on 7/15/2021 in the El Segundo Regional Office (ESRO) regarding the allegations referenced above. Upon arrival LPA met with Director Ashley Accosta and was guided on a tour of the facility. This facility houses an infant and preschool license. LPA observed the following children in the preschool: Dolphins class with 25 children and three staff, Sea Star class with 16 children and three staff, Sea Stars on the playground with 12 children and two staff, Sea Horse class with 17 children and three staff, and Sea Horse class on the playground with 10 children and one staff.

During the course of this investigation, LPA conducted interviews with the Reporting Party, staff, and parents. LPA conducted playground inspection and found no debris, reviewed children's files and facility Daily Cleaning Checklist. The Daily Cleaning Checklist lists "rake sandbox" but does not refer to turf area. Per facility, when raking sandbox, a walk through of the turf area is also conducted at least two to three times per day.

Investigator T. Brunelli from the Investigations Bureau also conducted interviews with the Reporting Party, Licensee, and staff. LPA received no pictures of bruising on child. LPA did receive a picture of glass but LPA was unable to determine the origin of the glass. Through interviews, LPA was unable to meet the preponderance of evidence to prove the allegations occurred nor was LPA able to find substantial evidence to show that the allegations were false and could not have happened. Therefore, the allegations are Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the allegations occurred.

An exit interview was conducted with Director Ashley Accosta, CEO Denise Tyner attended via phone call.

A copy of this report was read and discussed with Director Accosta and CEO Tyner. A copy of this report along with the Notice of Site Visit will be provided to Director Accosta.
The Notice of Site Visit is to be posted in a prominent area for 30 days.
