 On 03/23/2023 Licensing Program Analyst( LPA) Doris Whitmore conducted an unannounced Case Management for the purpose of following up an Unusual Incident Report( UIR) that occurred on 01/24/2023. Upon arrival LPA met with Michelle Regalado and informed the nature of the visit. There was a total of 45 children and nine teachers. LPA spoke to Therapist to follow up regarding permission to interview him and stated the card was given to his Supervisor. The Education Coordinator Michelle Regalado followed up and tried to contact the Supervisor the mailbox was full she was unable to leave a message. LPA will come back to continue and follow up with the Unusual Incident. Exit interview was conducted Notice of Site Visit along the Appeal Rights were given.
