LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Physical Plant - Licensee does not ensure ceiling is in good repair. LPA observed a Work Order that had been submitted due to the recent heavy rain, which had caused leakage. LPA did not observe any discolored tiles in the ceiling. Personal Rights - Licensee does not ensure day care children are free from lice before accepting. During interviews staff demonstrated and explained to LPA how if needed at the time of wellness check, how they instruct parents on use of the zapper comb for lice, if children show signs. Parents received exposure letters and/or treatment forms. Personal Rights - Licensee does not ensure toys are in good condition for day care children. LPA checked indoors and outdoors and did not observe any toys that were not in good condition. Physical Plant - Licensee does not ensure outdoor play area is free from hazards, during interviews staff stated that there is a daily check of the interior and exterior of the facility prior to children’s arrival for any hazards and a checklist is completed. Personal Rights - Staff do not prevent day care children from harming other children. During interviews it was stated that if there is an issue with children not getting along, the children are separated and redirected provided with activities.
Therefore, the allegations are deemed unsubstantiated. Meaning, although the allegations
may have happened or valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. No deficiencies cited.
An exit interview was conducted, copy of this report was read, appeal rights along with Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days.
