13 | On 12/04/2025 Licensing Program Analyst, LPA, Tyra Chavies, met with director, Isis Broaster, for the purpose of an unannounced Case Management visit to deliver complaint findings. There were 20 children being supervised by 3 staff members.
On 09/27/25 LPA Chavies conducted telephone interviews with P#2 and P#4
On 09/24/25 LPA Chavies attempted to conducted interviews with P#2, P#3 and P#4
On 09/18-19/25 LPA Chavies conducted a telephone interview with RP
On 09/18/25 LPA Chavies conducted in person interviews with Director #1 and Director #2
On 09/18/25 LPA Chavies observed children in care obtained a copy of the facility roster, personnel roster and facility documents.
Based on LPA's observation, record review, interviews conducted, the preponderance amount of evidence standard has not been met. Therefore, the above allegation, Staff did not report child's injuries to parent or authorized representative, is found to be UNSUBSTANTIATED. |
